Schedule An Appointment
Scheduling an appointment with Collaborating Minds LLC is simple! There is a “Request Appointment Now” button at the top right of the website. Once you click on that button, it takes you to the online scheduler. Here, you will enter your demographic and other requested information. The appointment is not confirmed at that time. This is an appointment request. I will confirm the appointment if the opening is available. If there are any schedule conflicts, I will call, text or email you to discuss an alternative date and time that works best for you.
**Intake forms must be completed at least 24 hours prior to your appointment. Please check your email or text for intake forms and complete them (including uploading your insurance card) as soon as possible. If this is not completed, your appointment will be rescheduled.
Once your appointment is confirmed, you will receive a telemedicine link via email or text. You will utilize this link to join the session on the day of your appointment. For some reason if you do not receive the link, please let me know by calling 470-715-2433. Tip: Test your internet connection by logging in 5-7 minutes prior to your appointment time. The system will take you through a test to ensure your connection is able to withstand the telemedicine visit. If you are unable to connect with your cell phone, please use a laptop, tablet or desktop computer.
